Endoscopic Retrograde Cholangiopancreatography (ERCP) for common bile duct stones causes a systemic inflammatory response, indicated by increased cytokines and stress hormones. Non-invasive diagnostic methods are recommended prior to ERCP.

Background:

Purpose of the Study:

  • To evaluate the metabolic and inflammatory responses following ERCP in patients with common bile duct stones.
  • To assess changes in pro-inflammatory and anti-inflammatory cytokines, stress hormones, and acute-phase reactants post-ERCP.

Main Methods:

  • Prospective study of 50 patients diagnosed with common bile duct stones.
  • Measurement of serum cytokines (IL-1β, IL-6, IL-8, IL-12, IFN-γ, TNF-α, IL-4, IL-10, IL-13), stress hormones (ACTH, cortisol, GH, aldosterone), and C-reactive protein (CRP).
  • Blood samples collected 1 hour before ERCP, and 1 and 24 hours after the procedure.

Main Results:

  • Successful ERCP in all 50 patients.
  • Significant elevations in serum pro-inflammatory cytokines (except IL-13) and CRP levels were observed 1 and 24 hours post-ERCP.
  • Marked increases in ACTH, cortisol, and aldosterone levels were noted 1 and 24 hours after ERCP, while growth hormone (GH) showed no significant change.

Conclusions:

  • ERCP induces a systemic inflammatory response beyond the local site.
  • The procedure triggers significant metabolic and hormonal stress.
  • Non-invasive diagnostic approaches are advisable before undertaking ERCP to mitigate potential systemic effects.
