Background:

Extraskeletal osteosarcoma is a rare malignant mesenchymal neoplasm.
The small cell variant is exceptionally rare, posing diagnostic challenges.

Observation:

A 60-year-old woman presented with a large, painful left thigh mass after 18 months.
Magnetic resonance imaging revealed a complex lesion with cystic degeneration and necrosis.
Fine needle aspiration cytology indicated a small cell lesion with minimal osteoid.

Histopathological diagnosis confirmed small cell osteosarcoma involving adjacent muscles and fat.
Lymph nodes were spared in this case.
