Background:

  • Vascular complications in diabetes patients present diagnostic challenges.
  • Existing methods for predicting ischemic heart disease (IHD) in diabetes are insufficient.
  • A need exists for a simple, noninvasive screening tool for IHD in diabetic outpatients.

Purpose of the Study:

  • To develop and validate a predictive model for ischemic heart disease in patients with diabetes.
  • To identify key clinical and laboratory parameters for IHD risk assessment in this population.

Main Methods:

  • Analysis of five-year clinical data from 337 diabetic outpatients.
  • Inclusion of parameters such as ankle-brachial index (ABI), pulse wave velocity (PWV), aortic calcification, plaque score (PS), intima media thickness (IMT), electrocardiographic (ECG) changes, HbA1c, low-density lipoprotein cholesterol (LDL-C), uric acid (UA), urine albumin, age, sex, disease duration, and body mass index.
  • Multivariate logistic regression analysis and receiver operating characteristic (ROC) curve analysis.

Main Results:

  • Key predictors identified for IHD onset included ECG ischemic changes, aortic calcification, minimum ABI, maximum IMT, LDL-C, and UA.
  • The developed model demonstrated a high predictive accuracy with an area under the ROC curve of 0.879 (0.820 - 0.938).

Conclusions:

  • Ischemic heart disease in patients with diabetes can be effectively predicted.
  • A combination of conventional physical and laboratory tests provides a reliable screening method for IHD risk in diabetic patients.
