High-intensity training improved cardiovascular health in patients with substance use disorder (SUD). This intervention is feasible and reduces risks associated with SUD, including cardiovascular disease and premature death.

- No significant between-group differences were found for work economy, insomnia, or anxiety; however, the TG improved in depression scores.

- High-intensity training is a feasible intervention for SUD patients in clinical treatment.
- HIT offers a risk reduction for cardiovascular disease and premature death compared to conventional rehabilitation.
- Implementing HIT into SUD rehabilitation programs is recommended.
