Summary

Early detection of small hepatocellular carcinoma (HCC) and adequate hepatic resection are key for patient survival. Identifying tumors without portal vein thrombus or satellite nodules improves outcomes.

Area of Science:

  • Hepatobiliary Surgery
  • Surgical Oncology
  • Gastroenterology

Background:

  • Hepatocellular carcinoma (HCC) is a significant global health concern.
  • Early detection and appropriate treatment are crucial for improving patient survival rates.
  • Solitary HCCs smaller than 5 cm represent a specific subgroup requiring focused diagnostic and therapeutic strategies.

Purpose of the Study:

  • To identify key factors influencing patient survival in solitary hepatocellular carcinoma (HCC) smaller than 5 cm.
  • To evaluate the efficacy of different diagnostic modalities for small HCC detection.
  • To determine the impact of tumor characteristics and surgical interventions on HCC patient outcomes.

Main Methods:

  • Retrospective analysis of 32 patients with solitary HCC (<5 cm).

Related Experiment Videos

  • Comparison of ultrasonography, CT, and angiography for HCC detection based on tumor size.
  • Assessment of factors including tumor size, portal vein thrombus, satellite tumors, and surgical resection type (segmentectomy, subsegmentectomy).
  • Main Results:

    • Ultrasonography showed the highest detection rate for HCCs smaller than 2 cm.
    • Tumors larger than 2 cm were frequently associated with portal vein thrombus and/or satellite tumors.
    • The presence of portal vein thrombus and/or satellite tumors correlated with lower patient survival rates.

    Conclusions:

    • Early detection of small HCCs, particularly those lacking portal vein thrombus and satellite tumors, is vital.
    • Adequate hepatic resection, such as segmentectomy or subsegmentectomy, is a critical factor for enhancing HCC patient survival.
    • A combination of early diagnosis and appropriate surgical management significantly impacts long-term outcomes for HCC patients.
