  • To evaluate the clinical features and applicability of the International Classification of Headache Disorders, 3rd edition beta (ICHD-III beta) criteria for migraine aura status.
  • To compare ICHD-III beta criteria with a more liberal definition (≥3 aura episodes over three consecutive days).

Main Methods:

  • Systematic review of published migraine aura status cases.
  • Retrospective analysis of 500 migraine with aura cases.

Main Results:

  • Nine cases met the ICHD-III beta criteria for migraine aura status.
  • A more liberal definition identified a higher percentage of cases (4.2% vs. 1.7%).
  • Patients diagnosed with aura status, regardless of criteria, were older at onset, predominantly had visual symptoms, normal neuroimaging, and no sequelae.

Conclusions:

  • Both diagnostic criteria identify similar patient populations regarding demographics, symptoms, imaging, and outcomes.
  • Utilizing more liberal criteria may facilitate the diagnosis and therapeutic analysis of a larger cohort, potentially revealing distinct migraine aura status subtypes.
