Summary

Systemic sclerosis patients with normal nailfold videocapillaroscopy (NVC) show less skin and lung disease. This finding helps understand disease severity in systemic sclerosis (SSc) patients with normal NVC patterns.

Background:

  • Systemic sclerosis (SSc) typically presents with a specific nailfold videocapillaroscopy (NVC) pattern in 90% of cases.
  • This SSc-specific NVC pattern is often linked to disease severity and progression.

Purpose of the Study:

  • To characterize SSc patients exhibiting normal or nonspecific NVC findings.
  • To compare clinical and visceral involvement in SSc patients with normal/nonspecific NVC versus those with the SSc-specific NVC pattern.

Main Methods:

  • Retrospective cohort study design.
  • Comparison of 25 SSc cases with normal/nonspecific NVC against 63 SSc controls with the SSc-specific NVC pattern.
  • Analysis of clinical features and visceral organ involvement.

Main Results:

  • Normal/nonspecific NVC was significantly associated with the absence of skin sclerosis (32% vs. 6.3%, p=0.004).
  • Absence of telangiectasia (47.8% vs. 17.3%, p=0.006) and sclerodactyly (60% vs. 25.4%, p=0.002) were also linked to normal/nonspecific NVC.
  • Less frequent severe pulmonary involvement was observed in patients with normal/nonspecific NVC (26.3% vs. 58.2%, p=0.017).

Conclusions:

  • Normal/nonspecific NVC in SSc patients correlates with milder skin manifestations.
  • This NVC pattern is also associated with a reduced incidence of severe pulmonary complications in systemic sclerosis.
