Even with normal coronary arteries initially, elderly individuals can develop obstructive coronary artery disease (OCAD) later in life. Diabetes combined with advanced age or smoking increases OCAD risk in these patients.

- Evaluation of risk factors, including diabetes mellitus, age, and smoking, for OCAD development.

- 19% of patients initially with NCAs developed OCAD upon repeat catheterization.
- OCAD development occurred across all age groups studied.
- The combination of diabetes mellitus with age over 65 or with smoking significantly predicted OCAD development (P = .04).
Conclusions:
- Normal coronary arteries in elderly individuals do not preclude the future development of obstructive coronary artery disease.
- Specific risk factor combinations, notably diabetes with advanced age or smoking, are critical indicators for OCAD risk in this population.
