Summary

A treatment algorithm for pathologic nipple discharge effectively identifies patients for safe clinical follow-up. Most cases resolve without surgery, avoiding unnecessary procedures.

Background:

  • Nipple discharge affects 2-5% of women.
  • A treatment algorithm was previously proposed for patients with nipple discharge.

Purpose of the Study:

  • To evaluate the effectiveness of a proposed treatment algorithm for pathologic nipple discharge.
  • To determine the risk of carcinoma in patients with nipple discharge.

Main Methods:

  • Patients with pathologic nipple discharge and negative mammogram/ultrasound underwent follow-up (2005-2011).
  • The algorithm guided management, including subareolar excision or clinical follow-up.
  • Carcinoma rates were analyzed based on diagnostic and management pathways.

Main Results:

  • The overall carcinoma risk was 5% in 192 patients (mean age 56).
  • Carcinoma rates varied: 17% after subareolar excision, 1% with clinical follow-up.
  • Subareolar excision was performed in 21% of follow-up patients due to imaging abnormalities or persistent discharge.

Conclusions:

  • Patients with nipple discharge can be safely managed with clinical follow-up based on radiographic and clinical findings.
  • Most patients with nipple discharge will experience resolution, avoiding surgical intervention.
