Women with a unicornuate uterus and rudimentary horn can achieve successful pregnancy. Surgical excision of the rudimentary horn and close monitoring are recommended for improved obstetric outcomes.
Area of Science:
- Reproductive Medicine
- Gynecologic Surgery
- Mullerian Duct Anomalies
Background:
- Unicornuate uterus with a non-communicating rudimentary horn is a rare Mullerian anomaly.
- This condition is associated with significant gynecological and obstetric complications, including infertility and preterm delivery.
- Pregnancy management in these cases remains challenging due to poor reproductive outcomes.
Observation:
- A case report of a 26-year-old woman with a unicornuate uterus and rudimentary horn is presented.
- Diagnosis was confirmed using 2D and 3D sonography.
- Laparoscopic excision of the symptomatic rudimentary horn and ipsilateral fallopian tube was performed.
Findings:
- The patient experienced a normal fetal growth.
- Cervical shortening at 20 weeks necessitated tocolytic therapy.
- Successful cesarean delivery was achieved at 39 weeks and 4 days' gestation.
Implications:
- Successful pregnancy is achievable despite a historically poor reproductive prognosis for unicornuate uterus.
- Laparoscopic excision of the rudimentary horn during the non-pregnant state is advised.
- Serial ultrasound monitoring for fetal growth and cervical length is crucial for high-risk pregnancies.
