Abnormal neurons in ovarian teratomas distinguish N-methyl-d-aspartate receptor (NMDAR) encephalitis cases. These findings in teratomas may drive the development of autoimmune encephalitis.

- Abnormal neuroglial elements were closely associated with immune infiltrates in teratomas from NMDAR encephalitis cases (4/4).
- Inflammatory infiltrates were not associated with neuroglial tissue in control teratomas (P < .001).
Conclusions:
- The presence of abnormal neurons within ovarian teratomas reliably distinguishes NMDAR encephalitis cases.
- These abnormal neuroglial elements may play a role in triggering autoimmunity in NMDAR encephalitis.
- Findings highlight the importance of pathological examination of teratomas in autoimmune encephalitis.
