  • Non-invasive methods like transient elastography (TE) and point shear-wave elastography (PSWE) are increasingly used.

Purpose of the Study:

  • To evaluate the validity of point shear-wave elastography (PSWE) for liver stiffness assessment.
  • To determine the reproducibility and accuracy of PSWE compared to liver biopsy (LB) and transient elastography (TE).

Main Methods:

  • A single-center, cross-sectional study involving 134 patients with chronic viral hepatitis and 69 healthy volunteers.
  • Point shear-wave elastography (PSWE) and transient elastography (TE) were performed on all subjects.
  • Patients underwent ultrasound-assisted liver biopsy (LB) for staging fibrosis (METAVIR score); PSWE reproducibility was assessed by two expert raters.

Main Results:

  • PSWE showed high correlation with fibrosis stage, similar to TE.
  • Area under the receiver operating characteristic curve (AUC) values for PSWE and TE were comparable for detecting significant fibrosis (F ≥ 2, F ≥ 3, F = 4).
  • PSWE demonstrated excellent intraobserver (0.83-0.96) and interobserver (0.83-0.93) reproducibility.

Conclusions:

  • Point shear-wave elastography (PSWE) is a reproducible and accurate method for assessing liver stiffness.
  • PSWE performance is comparable to transient elastography (TE) in staging liver fibrosis.
  • Healthy volunteers exhibited significantly lower liver stiffness values compared to patients with non-significant fibrosis.
