    Summary

    Scleromyxedema diagnosis may not require paraprotein identification. This case study supports scleromyxedema as a distinct condition independent of gammopathy, challenging traditional diagnostic criteria.

    Background:

    • Scleromyxedema diagnosis traditionally relies on identifying abnormal paraproteinaemia.
    • Paraproteins are abnormal proteins produced by plasma cells, often associated with certain cancers or autoimmune diseases.

    Observation:

    • A case of scleromyxedema in a 51-year-old male is presented.
    • This patient showed no immunochemical evidence of paraprotein, a key diagnostic marker.

    Findings:

    • The absence of paraprotein in this case challenges the necessity of its detection for scleromyxedema diagnosis.
    • Clinical and experimental data suggest scleromyxedema can be independent of gammopathy.
