    Summary

    A rare superior cerebellar artery (SCA) aneurysm caused a subarachnoid hemorrhage in a young woman. Surgical clipping led to a full recovery, highlighting good prognosis for peripheral SCA aneurysms.

    Background:

    • Peripheral aneurysms of the superior cerebellar artery (SCA) are uncommon vascular lesions.
    • Subarachnoid hemorrhage (SAH) presents a significant risk of neurological deficit and mortality.

    Observation:

    • A 37-year-old female presented with severe headache and somnolence, diagnosed with SAH.
    • Initial angiography was negative, but a saccular left SCA aneurysm was identified on delayed imaging.
    • The patient developed cerebellar dysarthria, likely due to vasospasm, prior to successful surgical clipping.

    Findings:

    • A 2x3 mm saccular aneurysm of the left SCA was diagnosed 8 days after SAH onset.
    • Surgical clipping via an infratentorial-supracerebellar approach was performed on day 24.

  • The patient experienced an uneventful recovery and was discharged without neurological deficits.

    • Implications:

    • Peripheral SCA aneurysms should be considered in young patients presenting with SAH and focal neurological signs.
    • Early diagnosis and timely surgical intervention, even for peripheral lesions, can lead to favorable outcomes.
    • The infratentorial-supracerebellar approach is a viable surgical option for clipping peripheral SCA aneurysms.
