Background:

  • Ocular involvement in psoriasis is underrecognized, affecting approximately 10% of patients.
  • Ophthalmologic manifestations are more prevalent in arthropathic or pustular psoriasis and in males, often coinciding with disease exacerbations.

Purpose of the Study:

  • To determine the prevalence and characteristics of ocular involvement in psoriasis.
  • To compare ocular findings between psoriatic patients and healthy controls.
  • To evaluate the impact of a 12-week systemic immunosuppressive therapy on ocular and dermatological parameters.

Main Methods:

  • Thirty-two psoriatic patients and 32 healthy controls underwent dermatological (PASI, PGA, DLQI) and ophthalmological evaluations (Schirmer, Jones, BUT, DR-1 camera).
  • Ocular surface disease index and C-reactive protein (CRP) levels were assessed.
  • Patients received 12-week systemic immunosuppressive therapy.

Main Results:

  • Psoriatic patients exhibited significantly lower Schirmer, Jones, and BUT test values and more frequent conjunctival hyperemia compared to controls.
  • Ocular involvement was more pronounced in sebo-psoriasis, with significant correlations between PASI and Schirmer, Jones, and BUT tests.
  • After 12 weeks of therapy, significant improvements were observed in PASI, PGA, DLQI, CRP, and BUT values.

Conclusions:

  • Psoriasis is associated with a high rate of ocular involvement, particularly dry eye disease.
  • Sebo-psoriasis shows a stronger link with ocular surface parameters.
  • Systemic immunosuppressive therapy can ameliorate both psoriasis severity and ocular surface disease, underscoring the importance of routine ophthalmological screening for early detection and management.
