    Summary

    This study introduces single access retroperitoneal adrenalectomy (SARA) using a SILS port for adrenal tumors. The novel technique proved safe and feasible in its first national application.

    Background:

    • Adrenal incidentalomas require careful management.
    • Laparoscopic adrenalectomy is a standard surgical approach.
    • Novel minimally invasive techniques are being explored for adrenal surgery.

    Observation:

    • A 50-year-old male with a 38 mm right adrenal incidentaloma underwent a novel surgical procedure.
    • Single access retroperitoneal adrenalectomy (SARA) was performed using a multichannel SILS port through a small incision.
    • The procedure required an additional port for retraction and drainage.

    Findings:

    • The SARA was successfully completed without intraoperative complications.
    • The surgery took 120 minutes with 40 mL blood loss.
