Summary

Inflammatory breast cancer (IBC) is a rare, aggressive breast cancer. This study in India found IBC accounts for 1.1% of cases, with many presenting at advanced stages and showing high rates of hormone and HER2/neu receptor positivity.

Background:

  • Inflammatory breast cancer (IBC) is a rare, aggressive subtype of locally advanced breast cancer.
  • Characterized by rapid onset of erythema, edema, and peau d'orange, IBC presents unique diagnostic and therapeutic challenges.
  • Data on IBC in India are limited, necessitating local studies to understand its epidemiology and outcomes.

Purpose of the Study:

  • To evaluate the clinical-pathological characteristics of Inflammatory Breast Cancer (IBC) patients.
  • To determine the treatment outcomes for IBC at a major tertiary care center in India.
  • To provide data on the incidence and presentation of IBC within the Indian population.

Main Methods:

  • A retrospective review of 3,650 breast cancer cases registered between January 2004 and December 2012 was conducted.
  • 41 cases of IBC were identified and analyzed for demographic, clinical, radiological, and histopathological data.
  • Exclusion criteria included recurrent IBC or advanced breast cancer mimicking IBC.

Main Results:

  • IBC represented 1.1% of all breast cancer cases at the center, with a median patient age of 45 years.
  • A significant proportion (one-third) of patients presented with metastatic disease (AJCC Stage IV).
  • Receptor status revealed 50% Estrogen Receptor (ER) positivity, 46% Progesterone Receptor (PR) positivity, and 60% HER2/neu positivity, with 15% triple-negative cases.

Conclusions:

  • Inflammatory Breast Cancer (IBC) is a distinct entity with specific clinical-pathological features observed in the Indian population.
  • High rates of hormone receptor and HER2/neu positivity were noted, influencing treatment strategies.
  • The study highlights the need for increased awareness and tailored management approaches for IBC in India, given its aggressive nature and presentation at advanced stages.
