Rotation thromboelastometry and the hypercoagulable state in Cushing's syndrome

Rotation thromboelastometry (ROTEM®) showed altered clot initiation in Cushing's syndrome (CS) patients, despite normal deep vein thrombosis screening. This study evaluates ROTEM® for hypercoagulability in CS.

Purpose of the Study:
- To assess hypercoagulability in CS patients using ROTEM®.
- To correlate ROTEM® findings with standard coagulation markers.
- To investigate the association between ROTEM® parameters and deep vein thrombosis (DVT) in CS.
Main Methods:
- Thirty CS patients and 30 matched controls were enrolled.
- Coagulation markers including aPTT, platelets, fibrinogen, D-dimer, FVIII, vWF, and CRP were measured.
- ROTEM® analysis (INTEM, EXTEM, FIBTEM) and Doppler ultrasonography for DVT were performed.
Main Results:
- CS patients exhibited shorter INTEM clotting times and aPTT compared to controls.
- Elevated FVIII, vWF, and D-dimer levels were observed in CS patients.
- Obese CS patients showed specific ROTEM® alterations (INTEM MCF, alpha angle, speed; FIBTEM MCF); no DVTs were detected.
Conclusions:
- CS patients display altered clot initiation via ROTEM® and elevated FVIII/vWF levels.
- Despite hypercoagulability markers, no deep vein thrombosis was identified in the studied CS cohort.
- ROTEM® offers insights into coagulation dynamics in Cushing's syndrome.
