    Background:

    • Distal foot soft-tissue defects with exposed bone and tendon present significant reconstructive challenges.
    • Current reconstructive options may have limitations in achieving optimal aesthetic and functional outcomes.

    Observation:

    • This study evaluated the use of metatarsal artery perforator (MAP)-based propeller flaps for distal foot reconstruction.
    • Seven MAP-based propeller flaps were utilized in six patients between July 2011 and June 2012.

    Findings:

    • All seven MAP-based propeller flaps demonstrated complete survival.
    • Patients experienced excellent aesthetic and functional recovery, with all patients ambulating within one month post-surgery.
    • Minor transient distal venous congestion occurred in two patients, resolving spontaneously without complications.
