Background:

  • An emerging mechanism of burn injury involves the ignition of butane during the production of tetrahydrocannabinol (THC) concentrate, commonly known as butane honey oil.
  • The increasing popularity of butane honey oil manufacturing has led to a rise in severe burn incidents within the community.

Purpose of the Study:

  • To report a series of patients presenting with burn injuries resulting from butane honey oil production.
  • To describe the process causing these burns and analyze patient outcomes.

Main Methods:

  • Retrospective case series data collected from eight patients treated at University of California Davis Medical Center and Shriners Hospital of Northern California.
  • Information on butane honey oil manufacturing was gathered through internet searches and published literature.

Main Results:

  • Burn injuries ranged from 16% to 95% Total Body Surface Area (TBSA), averaging 49.9%.
  • Average patient stay included 118.3 hospital days, 114.4 ICU days, and 43.8 days on mechanical ventilation.
  • The average patient age was 22 years, with most patients under 30.

Conclusions:

  • Butane honey oil production poses a significant risk for severe burn injuries, particularly among young adults.
  • Burn care providers need to be aware of this emerging injury mechanism and its increasing prevalence.
