Summary

The acetone fat-clearing technique significantly increases lymph node retrieval in gastric cancer specimens compared to manual dissection. While more nodes were recovered, the number of positive nodes did not significantly differ between methods.

Area of Science:

  • Oncology
  • Surgical Pathology
  • Gastrointestinal Surgery

Background:

  • Lymph node yield in gastrectomy is crucial for accurate staging of gastric cancer.
  • Current methods for lymph node retrieval vary, impacting diagnostic accuracy.
  • Optimizing lymph node retrieval is essential for effective gastric cancer management.

Purpose of the Study:

  • To compare the efficacy of two lymph node retrieval techniques in gastric cancer specimens.
  • To evaluate manual dissection versus acetone fat-clearing for lymph node recovery.
  • To determine if technique impacts the number of harvested lymph nodes.

Main Methods:

  • Gastric cancer specimens were divided into two groups for lymph node retrieval.
  • Group 1: Manual dissection following formalin fixation.
  • Group 2: Acetone fat-clearing technique.

Main Results:

  • The acetone group yielded a significantly higher average number of lymph nodes (26.1) compared to the manual group (19.3) (P=0.003).
  • Demographic and pathological variables were comparable between groups.
  • The average number of positive lymph nodes showed a non-significant increase in the acetone group (6.9 vs 4.6).

Conclusions:

  • Acetone fat-clearing is a superior method for increasing lymph node yield in gastric cancer specimens.
  • This technique allows for a more comprehensive evaluation of lymph node status.
  • Further investigation is warranted regarding the clinical significance of the non-significant increase in positive nodes.
