The Thyroid Gland01:23

The Thyroid Gland

6.9K
The thyroid gland is a small, butterfly-shaped gland located in the neck and covers the anterior surface of the trachea. The gland has two lateral lobes connected by a thin tissue mass called the isthmus. Internally, each lobe comprises many small spherical structures known as thyroid follicles, surrounded by a network of blood vessels.
The follicles have a central cavity lined by simple cuboidal to squamous epithelial cells called follicular cells. These cells produce the glycoprotein...

Area of Science:

  • Endocrinology
  • Surgical Oncology
  • General Surgery

Background:

  • The ultrasonic scalpel utilizes high-frequency mechanical energy for simultaneous vessel sealing and tissue coagulation.
  • Traditional suture ligation techniques in thyroid surgery can be time-consuming and may lead to complications.

Purpose of the Study:

  • To compare the safety and efficacy of the ultrasonic scalpel (Ultracision) versus the Ligasure device in total thyroidectomy.
  • To evaluate operative time, drainage volume, complications, and hospital stay for both devices.

Main Methods:

  • A prospective study randomized 400 patients undergoing thyroidectomy into two groups.
  • Group A used the Ultracision device, while Group B used the Ligasure device.
  • Data collected included patient demographics, surgical outcomes, and postoperative parameters.

Main Results:

  • No significant differences were observed between the Ultracision and Ligasure groups regarding patient demographics, thyroid gland characteristics, or histopathologic findings.
  • Preoperative and postoperative serum calcium levels, complications, and reoperation rates were similar between the two groups.
  • Operative time and drainage volume showed no significant variations between the devices.

Conclusions:

  • Both the ultrasonic scalpel and Ligasure are safe, effective, and useful alternatives to traditional suture ligation in thyroid surgery.
  • These devices simplify total thyroidectomy by eliminating the need for manual clamp-and-tie maneuvers.
  • Efficient hemostasis and reduced operative time contribute to the benefits of these advanced surgical tools.
