Background:

  • Bronchiolitis is a leading cause of infant hospitalizations and healthcare costs.
  • The efficacy of nebulized hypertonic saline in the emergency department (ED) setting remains unclear despite previous studies suggesting potential benefits.

Purpose of the Study:

  • To evaluate the impact of nebulized 3% hypertonic saline versus 0.9% normal saline on hospital admission rates and length of stay in infants diagnosed with bronchiolitis.

Main Methods:

  • A double-blind, randomized clinical trial was conducted over three bronchiolitis seasons.
  • Infants under 24 months with viral bronchiolitis were randomized to receive either 3% hypertonic saline or 0.9% normal saline in the ED, with potential for repeated doses.
  • Exclusion criteria included prematurity, chronic pulmonary disease, immune deficiency, cardiac disease, or prior wheezing/bronchodilator use.

Main Results:

  • The study enrolled 197 patients in the normal saline group and 211 in the hypertonic saline group.
  • Hospital admission rates were significantly lower in the hypertonic saline group (28.9%) compared to the normal saline group (42.6%).

  • While hypertonic saline shows promise in reducing admissions, its effect on hospital length of stay and symptom severity (as measured by the Respiratory Distress Assessment Instrument) was not statistically significant in this study.
