    Area of Science:

    Background:

    • Orbital implant extrusion is a complication following evisceration.
    • Reconstruction of the orbital socket is necessary to restore function and aesthetics.

    Observation:

    • Three patients with extruded synthetic orbital implants underwent socket reconstruction.
    • An autogenous dermis fat graft was utilized, requiring an incision into the scleral fundus to create a vascular bed.
    • The anterior scleral ring with extraocular muscle attachments remained undisturbed.

    Findings:

    • Successful conjunctival reepithelialization of the dermal surface was observed.
    • Enhanced orbital volume was achieved in all cases.
    • Excellent globe motility was preserved post-reconstruction.
    • Graft viability, conjunctival integrity, and volume restoration remained stable at an 18-month follow-up.

    Implications:

    • Autogenous dermis fat grafts are a useful management option for orbital implant extrusion in previously eviscerated sockets.
    • This technique offers a durable solution for restoring orbital volume and function.
    • Preservation of globe motility is a significant advantage of this reconstructive approach.
