Background:

  • Central odontogenic fibroma (COF) is an uncommon odontogenic tumor, comprising 0.1% of all such neoplasms.
  • COF presents as a slow-growing lesion, potentially causing cortical expansion and presenting radiologically as unilocular or multilocular with possible root displacement or resorption.
  • Histopathological features include mature collagen fibers and abundant fibroblasts.

Observation:

  • A 49-year-old male presented with an asymptomatic central odontogenic fibroma of the mandible, discovered incidentally.
  • Orthopantomogram revealed generalized bone loss, a unilocular radiolucent area with a sclerotic border, and angular bone loss.
  • The patient was unaware of the lesion's presence due to lack of symptoms like swelling or pain.

Findings:

  • Surgical enucleation of the COF was performed.
  • The surgical defect was reconstructed using a bioresorbable demineralized bone matrix (DMBM) osseo-graft and platelet-rich fibrin.
  • Post-operative evaluation demonstrated excellent wound healing and significant bone height regeneration at the defect site.

Implications:

  • This case highlights the successful management of a rare central odontogenic fibroma through surgical intervention and bone grafting.
  • The use of DMBM and platelet-rich fibrin shows promise for effective bone regeneration in the management of odontogenic tumor defects.
  • Further research into COF is warranted due to its rarity and variable clinical presentation.
