    Summary

This study establishes normal pancreatic duct standards for Koreans using endoscopic retrograde cholangiopancreatography (ERCP). Findings detail duct morphology, diameter, length, and ampulla shapes for precise pancreatic disease diagnosis.

    Area of Science:

    • Gastroenterology and Hepatology
    • Medical Imaging
    • Anatomy

    Background:

    • Endoscopic retrograde cholangiopancreatography (ERCP) is crucial for diagnosing pancreatic diseases.
    • Precise delineation of normal pancreatography is essential for accurate ERCP interpretation.
    • Establishing normative data for pancreatic duct morphology in specific populations is needed.

    Purpose of the Study:

    • To define normal pancreatic ductal morphology (course, diameter, length) in the Korean population.
    • To characterize the shape of the ampulla in individuals with normal pancreatic status.
    • To determine the success rates of ERCP procedures for pancreatic and biliary duct visualization.

    Main Methods:

    • Analysis of pancreatograms from 449 individuals with proven normal pancreatic status.
    • Categorization of duodenal papilla shapes (hemispherical, papillary, flat).
    • Measurement of pancreatic duct dimensions (diameter, length) and assessment of ductal course.

    Main Results:

    • The hemispherical duodenal papilla shape was most common (49.7%).
    • Overall ERCP success rate was 95.3%, with selective success rates of 90.9% for the pancreatic duct and 79.3% for the biliary tree.
    • The ascending ductal course (51.7%) was most frequent; mean pancreatic duct diameter was 3.3mm (head), 2.4mm (body), and 1.5mm (tail), increasing with age. Mean length was 17.5cm.

    Conclusions:

    • This study provides essential normative data for pancreatic duct morphology in Koreans.
    • These parameters aid in the precise diagnosis of pancreatic diseases using ERCP.
    • The findings contribute to standardizing ERCP interpretation and improving diagnostic accuracy.
