Background:

  • Unhealthy behaviors contribute significantly to chronic disease, healthcare costs, and mortality.
  • The US Preventive Services Task Force recommends behavioral screening and intervention (BSI) services.

Purpose of the Study:

  • To assess patient satisfaction and behavioral outcomes of a program enabling healthcare settings to deliver BSI services.
  • To evaluate the effectiveness and cost-efficiency of paraprofessional-led BSI in primary care.

Main Methods:

  • A pre-post program evaluation was conducted across 31 primary care clinics and 2 other sites.
  • Over 110,000 patients completed multibehavioral screening; positive screens led to intervention by health educators.
  • Patient satisfaction was assessed via questionnaires (n>300), and behavioral outcomes were tracked through 6-month follow-up interviews (n=675). A pilot depression intervention involved 29 patients with 3-month follow-up.

Main Results:

  • Patient satisfaction scores averaged over 4.2 on a 5-point scale.
  • Significant reductions were observed: binge drinking episodes decreased by over 20%, marijuana use by 15%, and depression symptom scores by 55% over 6 months.

Conclusions:

  • Paraprofessionals, with training and support, can effectively deliver BSI services in healthcare settings.
  • This approach shows promise for population-wide management of behavioral health determinants and healthcare cost reduction.
