General practitioners (GPs) view primary prevention as their responsibility but selectively offer lifestyle advice. They fear damaging patient relationships with universal prevention strategies, suggesting a need for broader public health approaches.
Area of Science:
- Public Health
- General Practice
- Health Services Research
Background:
- Healthcare systems are reorienting towards primary prevention strategies.
- Understanding the role of general practitioners (GPs) and their attitudes towards primary prevention is crucial for guiding these efforts.
Purpose of the Study:
- To analyze the role of primary prevention in general practice.
- To assess general practitioners' (GPs) attitudes and practices regarding primary prevention.
Main Methods:
- A mixed-method study was conducted in Berlin, Germany, in 2011.
- Included a cross-sectional survey of 474 community-based general practitioners (GPs) and focus groups with 15 GPs.
- Utilized developed and tested survey and interview guidelines to assess beliefs, attitudes, and practices related to primary prevention.
Main Results:
- 70% of GPs considered primary prevention part of their responsibility.
- GPs recommended primary prevention (e.g., physical activity, healthy eating, smoking cessation) when indicated, but infrequently discussed alcohol consumption (25%) or smoking cessation (18%).
- GPs expressed concern about negative patient impact from uninvited health advice and viewed prevention as a shared responsibility with societal institutions.
Conclusions:
- Primary prevention, including lifestyle counseling, is not fully established in primary care.
- GPs employ a selective approach to preventive advice, prioritizing patient relationships over universal strategies.
- Multisectoral public health approaches may be more effective, benefiting patients with competing demands or limited resources for prevention programs.
