Summary

Mitral insufficiency, a common heart valve disease, can be treated with the MitraClip procedure for high-risk patients. While less effective than surgery, it offers mid-term functional improvement and good safety in experienced centers.

Area of Science:

  • Cardiovascular Medicine
  • Interventional Cardiology
  • Cardiac Surgery

Context:

  • Mitral insufficiency is a prevalent valvular heart disease.
  • Surgical mitral valve repair is the standard treatment.
  • Many patients are high-risk surgical candidates due to comorbidities.

Purpose:

  • To evaluate the MitraClip procedure as an alternative for mitral valve repair.
  • To assess the safety, efficacy, and mid-term outcomes of the MitraClip procedure.
  • To identify patient populations who may benefit from this minimally invasive approach.

Summary:

  • The MitraClip procedure offers a less invasive option for mitral valve repair, particularly for patients unsuitable for surgery.
  • While achieving a lesser degree of mitral insufficiency reduction compared to surgery, it demonstrates good safety in experienced centers and provides mid-term functional improvement.

  • Current evidence, primarily from registries with limited follow-up, suggests its consideration for symptomatic high-risk patients on optimal medical therapy.
  • Impact:

    • Provides a viable treatment option for high-risk patients with mitral insufficiency.
    • Highlights the need for further research, including randomized trials and longer follow-up, to refine the technique and equipment.
    • Suggests potential for future advancements in percutaneous combined procedures for valvular heart disease.
