  • Clinical examination revealed vertical gaze palsy and skew deviation, with a tendency to sway rightward while walking.

Findings:

  • Magnetic Resonance Imaging (MRI) of the brain identified an infarct in the dorsal midbrain at the superior colliculus level.
  • The infarct also involved the medial thalamus, correlating with the observed neurological deficits.

Implications:

  • This case underscores the utility of advanced neuroimaging in diagnosing uncommon cerebrovascular presentations.
  • Accurate localization of midbrain lesions is crucial for effective neurological management and understanding disease mechanisms.
