Summary

This case report details a rare instance of iridocorneal endothelial syndrome in a teenage male, presenting with unilateral vision loss and glaucoma. Early diagnosis and management are crucial for this uncommon condition.

Background:

  • Iridocorneal endothelial syndrome (ICES) is a spectrum of disorders characterized by abnormalities of the corneal endothelium, iris, and anterior chamber angle.
  • This condition can lead to secondary glaucoma and vision loss, necessitating prompt diagnosis and treatment.

Observation:

  • A 19-year-old male presented with a two-year history of reduced and fluctuating vision, pupillary shape changes, and iris color alteration in his left eye.
  • Ocular examination revealed unilateral stretch holes, iris atrophy, elevated intraocular pressure (36 mmHg) in the left eye, and peripheral anterior synechiae.
  • Specular microscopy demonstrated abnormal corneal endothelium with irregularly shaped cells.

Findings:

  • The patient exhibited signs consistent with iridocorneal endothelial syndrome, including unilateral corneal and iris abnormalities.
