Epistaxis01:30

Epistaxis

853
Epistaxis, or nosebleeds, occurs when small, swollen blood vessels in the nasal mucous membrane rupture. Typically, the anterior septum is the primary site of occurrence.
Etiology
Possible causes of this condition include high blood pressure, trauma, low humidity, upper respiratory tract infections, allergies, foreign bodies, nasal inhalation of corticosteroids or illicit drugs, excessive use of decongestant nasal sprays, facial or nasal surgery, anatomic malformation, tumors, or systemic...

Summary

Nasal skin cancers often require large reconstructions. Modifying the nose

Area of Science:

  • Dermatology
  • Plastic Surgery
  • Oncology

Background:

  • The nose is a common site for skin cancers, including basal cell carcinoma, squamous cell carcinoma, and melanoma.
  • Nasal defects from skin cancer excision can be wide, necessitating extensive reconstructive flaps and potentially noticeable scarring.
  • Aged noses often exhibit features like a plunging tip, increased length, and prominent humps, complicating reconstruction.

Observation:

  • Skin cancers on the nose, while often superficial, can present with significant width.
  • Reconstruction of nasal defects typically requires large flaps, leading to noticeable donor and recipient site scarring.
  • Pre-existing age-related nasal characteristics can influence defect size and reconstructive planning.

Findings:

  • Surgical reduction and modification of the nasal framework can downsize the defect.

  • Downsizing the defect reduces the amount of skin needed for coverage.
  • Open rhinoplasty facilitates maneuvers for framework reduction, simplifying reconstruction.
  • Implications:

    • Modifying the nasal framework can decrease the need for large flaps in nasal reconstruction.
    • Reduced flap size leads to smaller scars and potentially improved aesthetic outcomes.
    • Integrating framework reduction into skin cancer excision offers a more conservative reconstructive approach.
