Summary

Cortical blindness is a rare seizure symptom. This case shows how posterior partial seizures can cause temporary blindness and lasting visual problems.

Familial mesial temporal lobe epilepsy maps to chromosome 4q13.2-q21.3.

Neurology·2007

Area of Science:

  • Neurology
  • Ophthalmology
  • Epileptology

Background:

  • Cortical blindness is an uncommon ictal manifestation of epilepsy.
  • Partial seizures, particularly those originating in the occipital lobe, can present with complex neurological symptoms.

Observation:

  • A patient experienced intermittent visual impairment starting in adolescence.
  • Repeated partial seizures, especially occipital seizures, were associated with transient cortical blindness and other neurological deficits.
  • Following a cluster of occipital seizures, the patient developed permanent visual field defects.

Findings:

  • Neurological and imaging studies were initially normal despite continuous biposterior epileptiform discharges on EEG.
  • A new right occipital lesion was identified on CT scan after the onset of permanent visual deficits.

Related Experiment Videos

  • The case demonstrates a link between posterior partial seizures and both transient and permanent visual disturbances.
  • Implications:

    • This case highlights the importance of considering epilepsy, specifically posterior partial seizures, in the differential diagnosis of cortical blindness.
    • It suggests that prolonged or severe seizure activity in the occipital cortex can lead to irreversible visual pathway damage.
    • Further research into the mechanisms underlying seizure-induced visual deficits is warranted.
