Area of Science:

  • Psychology
  • Behavioral Medicine
  • Digital Health

Background:

  • Chronic pain presents a significant burden, impacting daily life and mental well-being.
  • Acceptance-based psychological interventions show potential for managing chronic pain.
  • Internet-delivered interventions offer scalable solutions for widespread access.

Purpose of the Study:

  • To evaluate an internet-delivered, guided self-help intervention based on Acceptance and Commitment Therapy (ACT) for chronic pain.
  • To compare the efficacy of ACT against an internet-based control (Expressive Writing) and a waiting list.

Main Methods:

  • A randomized controlled trial involving 238 chronic pain sufferers.
  • Participants were allocated to ACT, Expressive Writing, or a waiting list.
  • Outcomes were assessed at baseline, 3 months, and 6 months post-intervention.

Main Results:

  • ACT participants showed improved pain interference compared to Expressive Writing (Cohen's d = .47).
  • Adherence to ACT (48%) led to significant improvements in pain interference versus the waiting list (d = .49).
  • ACT also improved depression, pain intensity, psychological inflexibility, and pain catastrophizing (d: .28–.60), with 28% showing clinically relevant improvement.

Conclusions:

  • Internet-delivered ACT is a promising digital treatment modality for individuals with chronic pain.
  • The intervention demonstrated significant improvements in primary and secondary outcomes, including clinically relevant changes.
  • Adherence is a key factor for maximizing the benefits of online ACT for chronic pain management.
