Background:

Bipolar disorder treatment often involves lithium, but some patients are refractory.
Anticonvulsant medications are increasingly explored for mood stabilization.

Observation:

Carbamazepine and valproate represent novel therapeutic options for bipolar disorder.
These anticonvulsants appear effective in a subset of patients, especially those with rapid cycling bipolar illness.

Findings:

Preliminary evidence suggests selective patient response to either carbamazepine or valproate.
Different antiepileptic profiles and mechanisms of action may explain differential efficacy.

Implications:

Further research is needed to elucidate shared mechanisms between anticonvulsant and psychotropic effects.
Valproate's role in acute and long-term bipolar disorder management requires controlled evaluation.
Individualized treatment strategies may be beneficial for patients with lithium-refractory bipolar disorder.
