Giant Gartner duct cysts are rare remnants of the mesonephric ducts. Measuring cyst fluid CA-125 may aid in diagnosing these rare cystic lesions.

Area of Science:

  • Urology
  • Gynecologic Oncology
  • Pediatric Surgery

Background:

  • Gartner duct cysts are rare, vestigial remnants of the mesonephric (Wolffian) ducts.
  • Typically solitary and less than 2 cm, giant Gartner duct cysts are exceptionally uncommon.
  • These cysts can be associated with renal and ureteral abnormalities.

Observation:

  • A case of a giant Gartner duct cyst in a young female is presented.
  • The cyst was successfully treated with surgical excision.
  • Preoperative aspiration showed elevated CA-125 levels in the cyst fluid, contrasting with normal serum levels.

Findings:

  • The giant Gartner duct cyst in this case had no associated pathological findings.
  • Cyst fluid analysis revealed significantly elevated CA-125.

  • Serum CA-125 levels remained within the normal range.
  • Implications:

    • Elevated cyst fluid CA-125 may serve as a diagnostic marker for Gartner duct cysts.
    • This finding could refine the diagnostic approach to cystic lesions in the pelvic region.
    • Further research may elucidate the role of CA-125 in differentiating cystic pathologies.
