Summary

Self-etch (SE) adhesives resulted in less postoperative sensitivity at 24 hours compared to total-etch (TE) adhesives for composite restorations in Class-V cavities. This finding suggests SE adhesives may help reduce immediate post-operative discomfort.

Background:

Postoperative sensitivity is a common concern following dental restorations.
Adhesive systems, including self-etch (SE) and total-etch (TE) types, play a crucial role in bonding composite resins to tooth structure.
Understanding the impact of different adhesive systems on postoperative sensitivity is essential for clinical decision-making.

Purpose of the Study:

To compare the level of postoperative sensitivity in Class-V cavities restored with composite using either a self-etch adhesive or a total-etch adhesive.
To evaluate the efficacy of SE adhesives in mitigating immediate postoperative sensitivity compared to TE adhesives.

Main Methods:

A randomized clinical trial involving 70 patients with Class-V carious lesions.

Cavities were prepared and restored using either SE adhesive (adheSE one) or TE adhesive (Eco-Etch) followed by light-cured composite (Te-Econom).

Postoperative sensitivity was assessed using a visual analogue scale at baseline, immediately after restoration, 24 hours, and 1 week post-procedure, with cold stimulus and compressed air.

Main Results:

A statistically significant difference in sensitivity was observed between the SE and TE groups at the 24-hour follow-up using a cold stimulus.
No significant differences in sensitivity were found at baseline, immediately after restoration, or at the 1-week follow-up for either cold stimulus or compressed air.

Conclusions:

Restorations placed with self-etch adhesives demonstrated less postoperative sensitivity at the 24-hour assessment compared to those placed with total-etch adhesives.
The findings suggest that self-etch adhesives may be beneficial in reducing postoperative sensitivity within the first 24 hours after composite restoration placement in Class-V cavities.
