Pharmacological maintenance treatment for adolescents shows promise in retaining them in care and reducing opioid use. However, more research is urgently needed due to limited evidence from just two trials.
Area of Science:
- Addiction Medicine
- Adolescent Health
- Pharmacotherapy
Background:
- Pharmacotherapy is crucial for opioid-dependent adults.
- Limited systematic reviews exist on pharmacological maintenance treatment for adolescents.
Purpose of the Study:
- To assess the effectiveness of maintenance treatments for adolescent opioid dependence.
- Evaluated retention in treatment, substance use reduction, and health/social status improvement.
Main Methods:
- Systematic review of randomized controlled trials.
- Searched multiple databases up to January 2014.
- Included trials comparing maintenance treatment with other interventions in adolescents (13-18 years).
Main Results:
- Two trials (189 participants) were included: methadone vs. LAAM, and buprenorphine-naloxone maintenance vs. detoxification.
- Buprenorphine maintenance improved treatment retention and reduced self-reported opioid use.
- Evidence quality was low to very low; no serious side effects reported.
Conclusions:
- Limited evidence from two trials makes definitive conclusions difficult.
- Further randomized controlled trials with long follow-up are urgently needed.
- Future research should compare maintenance with detoxification or psychosocial treatments.
