Merkel cell carcinoma (MCC) shows inflammatory cells at tumor margins, but these CD8+ T cells and macrophages cannot penetrate the tumor microenvironment. This limited immune cell infiltration hinders an effective anti-tumor response against Merkel cell carcinoma.

Background:
- Merkel cell carcinoma (MCC) is an aggressive skin cancer linked to aging, immunodeficiency, and Merkel cell polyomavirus (MCPyV).
- Intra-tumoral CD8+ T cells are associated with improved MCC survival.
Purpose of the Study:
- To characterize the distribution and properties of inflammatory cells, particularly CD8+ T lymphocytes, within primary MCC specimens.
- To investigate the interaction between immune cells and tumor cells expressing MCPyV large T antigen.

- CD8+ cells were located at the tumor edge and within septa, but did not contact MCC cells expressing MCPyV large T antigen.
- CD8+ cells expressed neither CXCR3 nor granzyme B, indicating an inability to infiltrate the tumor microenvironment effectively.
Conclusions:
- Inflammatory cells accumulate around MCC deposits, suggesting a response to tumor signaling.
- The tumor microenvironment prevents immune cell penetration, limiting the anti-tumor immune response in Merkel cell carcinoma.
- Further research is needed to understand and overcome these immune evasion mechanisms in MCC.
