Background:

  • Fear of falling is a significant concern for individuals with multiple sclerosis (MS).
  • Associations between gait parameters and fear of falling in MS are not well-established.

Purpose of the Study:

  • To investigate the relationship between spatio-temporal gait parameters and fear of falling in people with MS.

Main Methods:

  • A cross-sectional study involving 130 MS patients and 25 healthy controls.
  • Gait analysis using the GAITRite system and assessment of fall concerns with the Falls Efficacy Scale International (FES-I).
  • Participants were categorized into 'more concerned' (>20) and 'less concerned' (≤20) groups based on FES-I scores.

Main Results:

  • Individuals with higher fear of falling exhibited slower walking speed, shorter step length, prolonged double support, wider base of support, and shorter single support.
  • Gait parameters explained a significant portion of the variance in fear of falling (40.3%).
  • Spatial gait components accounted for 30.9%, temporal components increased this to 36.7%, and asymmetry parameters further enhanced the model.

Conclusions:

  • Quantitative evidence confirms the link between spatio-temporal gait performance and fear of falling in MS.
  • Gait parameters can help assess fear of falling in MS patients.
  • Step length may serve as a surrogate outcome for interventions targeting fear of falling reduction in MS.
