Urinary Tract Calculi VI: Surgical Management

1.0K
Procedures for Kidney StonesMedical intervention is necessary when kidney stones or renal calculi are too large to pass spontaneously (typically greater than 5 millimeters) when stones are accompanied by symptomatic infection (such as fever or pyelonephritis), when they impair kidney function, or when they cause persistent symptoms like severe pain, nausea, or urinary retention. Additionally, patients with only one kidney or those who cannot be treated with medical management also require...

Summary

This study reviews surgical management for rectovaginal fistulas, finding obstetric trauma is a common cause. Conservative fistulectomy with flap repair shows good results for low fistulas.

Area of Science:

  • Colorectal Surgery
  • Gynecologic Surgery
  • Surgical Management of Fistulas

Background:

  • Rectovaginal fistula is an abnormal connection between the rectum and vagina.
  • Etiologies include obstetric trauma, foreign bodies, and surgical complications.

Purpose of the Study:

  • To share clinical experience and surgical management strategies for rectovaginal fistulas.
  • To evaluate outcomes of different surgical techniques.

Main Methods:

  • Retrospective study of 15 patients treated for rectovaginal fistulas between 2005 and 2011.
  • Surgical interventions were tailored to fistula etiology, size, and location.

Main Results:

  • The most common cause was obstetric trauma.
  • Common symptoms included passage of gas and stool from the vagina.
  • Postoperative complications included wound infection and recurrence in two patients each.

Conclusions:

  • Rectovaginal fistula treatment is complex due to anatomical variations.
  • Conservative fistulectomy with advancement flaps is effective for low rectovaginal fistulas.
  • Surgical technique selection depends on individual patient factors.
