Glaucoma: Overview01:25

Glaucoma: Overview

1.7K
Glaucoma is an eye condition characterized by increased intraocular pressure that damages the retina and optic nerve, leading to irreversible blindness if left untreated. The human eye has various components, including the cornea, iris, pupil, lens, and optic nerve. Aqueous humor is secreted by the epithelium of the ciliary body in the posterior chamber and flows through the trabecular meshwork and canal of Schlemm, maintaining normal intraocular pressure. The trabecular meshwork and the canal...

Open Angle Glaucoma: Treatment01:27

Open Angle Glaucoma: Treatment

1.0K
In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. The treatment for open-angle glaucoma focuses on reducing the elevated intraocular pressure by either decreasing the secretion of aqueous humor or increasing its outflow.
Drugs such as carbonic anhydrase inhibitors, α2- and...

Summary

Visual field testing in open-angle glaucoma patients generally does not increase intraocular pressure. IOP reduction after testing correlates with baseline IOP and central corneal thickness.

Area of Science:

  • Ophthalmology
  • Glaucoma Research
  • Intraocular Pressure Monitoring

Background:

  • Open-angle glaucoma (OAG) is a leading cause of irreversible blindness.
  • Accurate intraocular pressure (IOP) measurement is crucial for managing OAG.
  • Visual field (VF) testing is a standard diagnostic tool in glaucoma management.

Purpose of the Study:

  • To investigate the effect of visual field (VF) testing on intraocular pressure (IOP) in patients with open-angle glaucoma (OAG).
  • To determine if VF testing causes a significant alteration in IOP in OAG eyes.

Main Methods:

  • A prospective clinical trial involving 106 treatment-naive OAG patients.
  • Intraocular pressure (IOP) and refractive error were measured before and after VF testing using non-contact tonometry and auto refractometry.
  • Measurements were performed on both the right (OD) and left (OS) eyes.

Main Results:

  • The average baseline IOP was approximately 12.8 mmHg (OD) and 12.6 mmHg (OS).
  • VF testing on the right eye (OD) resulted in a statistically significant decrease in IOP (P=0.001), while the left eye (OS) showed no significant change (P=0.190).
  • Multivariate analysis revealed significant correlations between IOP decrease and baseline IOP, and central corneal thickness.

Conclusions:

  • Visual field (VF) testing does not typically increase intraocular pressure (IOP) in most open-angle glaucoma (OAG) patients.
  • The degree of IOP reduction post-VF testing is significantly associated with baseline IOP and central corneal thickness.
