Idiopathic lumbosacral plexitis (LSP), a rare immune disorder, causes sudden leg pain and numbness. Prompt steroid treatment led to significant symptom improvement in a patient with this condition.

Background:
- Idiopathic lumbosacral plexitis (LSP), also known as non-diabetic lumbosacral radiculoplexus neuropathy (LRPN), is a rare immune-mediated disorder.
- It affects multiple lumbosacral nerve roots and plexus without trauma, mass lesions, or high blood sugar.
Observation:
- A 60-year-old male presented with acute onset of left hip pain radiating down the leg.
- Associated symptoms included numbness on the lateral dorsum of the left foot over 4 days.
Findings:
- MRI of the lumbosacral plexus showed gadolinium enhancement, predominantly involving the left L5 nerve root.
- The patient experienced a dramatic clinical response to oral steroid therapy.
Implications:
- This case highlights the diagnostic considerations for acute lumbosacral radiculoplexus neuropathy.
- Early recognition and treatment with corticosteroids may lead to favorable outcomes in idiopathic LSP.
