Summary

Pelvic incidence, a morphologic parameter, accurately predicts acetabular cup orientation. This finding suggests pelvic incidence can define a safe range of pelvis tilt for individuals, improving surgical outcomes in orthopaedics.

Area of Science:

  • Orthopaedics
  • Anatomy
  • Biomechanical Engineering

Background:

  • Acetabular cup orientation is crucial in orthopaedics, traditionally relying on pelvis tilt.
  • Pelvic tilt is a positional parameter, while pelvic incidence is a morphologic parameter.
  • A more reliable method for assessing safe acetabular orientation is needed.

Purpose of the Study:

  • To develop a predictive equation for acetabular 3D orientation using pelvic incidence.
  • To investigate the relationship between pelvic incidence and acetabular orientation.
  • To account for asymmetry between right and left acetabulae.

Main Methods:

  • Measurement of 12 pelvic anatomic specimens using an electromagnetic Fastrak system.
  • Correlation analysis using a Spearman matrix for acetabulum and pelvis data.
  • Robust linear regression analysis to predict acetabular axes.

Main Results:

  • Acetabular orientation can be accurately predicted by pelvic incidence.
  • Pelvic incidence is correlated with acetabular morphology.
  • Asymmetry of the acetabular roof is also correlated with pelvic incidence.

Conclusions:

  • Pelvic incidence offers a more accurate and reliable assessment of acetabular orientation than pelvis tilt.
  • Pelvic incidence can determine an individual's safe range of pelvis tilt.
  • This provides a personalized approach to acetabular surgery planning.
