Vascular compression of the facial and intermediate nerves caused hemifacial spasm and otalgia in a patient. Microvascular decompression effectively treated these symptoms, confirming the cause.

Background:

- A 55-year-old woman presented with a decade of HFS and a month of ipsilateral otalgia.
- MRI revealed vascular loops adjacent to the facial nerve root.
Findings:
- Surgical exploration identified anterior inferior cerebellar artery (AICA) loops compressing the facial and intermediate nerves.
- Microvascular decompression (MVD) resulted in complete resolution of both HFS and otalgia.
Implications:
- This case highlights vascular compression of the intermediate nerve as a cause of paroxysmal otalgia.
- MVD is an effective treatment for intermediate nerve neuralgia, particularly when co-occurring with HFS.
