Summary

A nonblast corneal injury caused a unique ring-shaped disruption of endothelial cells, visualized with corneal optical coherence tomography (OCT). The injury resolved spontaneously, demonstrating transient stromal edema.

  • Corneal endothelial rings are rare, often associated with blast injuries.
  • Nonblast mechanisms for such injuries are less understood.
  • Optical coherence tomography (OCT) is a valuable tool for visualizing corneal structures.

Observation:

  • A 59-year-old male presented with a grey-white, annular corneal lesion after a nonpenetrating foreign body injury.
  • Slit-lamp examination revealed a 3.3 mm diameter endothelial ring.
  • Corneal OCT demonstrated stromal edema and endothelial disruption.

Findings:

  • The corneal OCT findings confirmed a traumatic corneal endothelial ring.
  • The observed pattern suggests a concussive injury mechanism.
  • Stromal edema was localized anterior to the disrupted endothelial cells.

Implications:

  • This case highlights a novel presentation of traumatic corneal injury.
  • Corneal OCT is effective in diagnosing and monitoring such conditions.
  • Findings support animal study hypotheses on injury mechanisms and sequelae.
