Summary

Exposure to straight metalworking fluids (MWFs) significantly increases bladder cancer risk. Jobs with potential mineral oil exposure also elevated risk, strengthening evidence of mineral oil as a carcinogen.

Background:

  • Metalworking is linked to bladder cancer, with metalworking fluids (MWFs) suspected as the exposure source.
  • Limited epidemiologic data exists on MWF exposure and bladder cancer risk.
  • This study addresses this gap using quantitative exposure assessment.

Purpose of the Study:

  • To investigate the association between metalworking fluid exposure and bladder cancer risk.
  • To quantify the risk associated with different types of MWFs.
  • To assess the role of mineral oil exposure in bladder cancer.

Main Methods:

  • A case-control study involving 895 male bladder cancer cases and 1031 population controls.
  • Detailed occupational histories were collected, focusing on the use of straight, soluble, and synthetic MWFs.
  • Exposure probability, frequency, intensity, and cumulative exposure were estimated and analyzed using logistic regression, adjusting for smoking and other factors.

Main Results:

  • Significant bladder cancer risk increase for men using straight MWFs (OR=1.7).
  • A significant trend of increasing risk with cumulative exposure to straight MWFs was observed (p=0.024).
  • Increased risk was also noted for jobs with potential mineral oil exposure, even in non-metalworkers.

Conclusions:

  • Exposure to straight metalworking fluids is a significant risk factor for bladder cancer.
  • Employment in jobs with potential mineral oil exposure also elevates bladder cancer risk.
  • These findings reinforce the evidence of mineral oil as a bladder carcinogen.
