Skin cancer is a type of cancer that occurs when there is an abnormal growth of skin cells, usually triggered by damage to the DNA within the skin cells. Summary

A rare Merkel cell carcinoma of the chest wall was successfully treated with wide local excision. This aggressive cancer showed no recurrence after four years of follow-up, highlighting the effectiveness of surgical management.

Background:

  • Merkel cell carcinoma (MCC) is a rare, aggressive skin cancer with a predilection for sun-exposed areas.
  • Chest wall MCC is exceptionally uncommon, posing diagnostic and therapeutic challenges.

Observation:

  • A 33-year-old male presented with a 4-month history of a bleeding ulcerated mass on the right anterior axillary fold.
  • The mass measured 2 cm in diameter with central ulceration and adherent clot.
  • No regional lymphadenopathy was detected.

Findings:

  • Histopathological examination revealed a malignant small round-cell tumor.
  • Immunohistochemistry confirmed cytokeratin-20 positivity and S100 negativity, consistent with Merkel cell carcinoma.
  • Wide local excision with a 2 cm margin was performed.

Implications:

  • Complete surgical resection may be curative for localized Merkel cell carcinoma of the chest wall.
  • This case underscores the importance of considering rare diagnoses in unusual locations.
  • Long-term surveillance is crucial for detecting potential recurrence of this aggressive malignancy.
