Summary

Disseminated IgG lambda myeloma can cause unusual corneal deposits. These translucent, subepithelial deposits were found to be composed of the same IgG lambda paraprotein present in the patient's serum and tears.

Observation:

  • A 46-year-old female with disseminated IgG lambda myeloma presented with unique translucent deposits in the peripheral corneas.
  • These subepithelial deposits were bilateral.

Findings:

  • Electrophoretic analysis confirmed the corneal deposits comprised an IgG lambda paraprotein.
  • This paraprotein was identical to the one identified in the patient's serum.
  • Trace amounts of the paraprotein were also detected in the patient's tears.

Implications:

  • This case highlights a rare ocular manifestation of multiple myeloma.
  • Corneal deposits can serve as a potential diagnostic marker for paraproteinemias.
  • Understanding the composition of these deposits aids in diagnosing and managing myeloma-related ocular complications.
