Summary

Primary pancreatic leiomyosarcomas are rare, aggressive tumors. Consider this diagnosis for large pancreatic masses with cystic-necrotic features, especially after ruling out other conditions.

Background:

Primary pancreatic leiomyosarcomas are exceptionally rare malignant neoplasms.
These tumors typically present with an aggressive clinical course and large dimensions.

Observation:

A 56-year-old female patient reported an 8-year history of abdominal pain.
Multislice computed tomography identified a substantial, heterogeneous pancreatic mass exhibiting necrotic, calcified, and macroscopic fatty components.

Findings:

Surgical excision and subsequent histopathological examination confirmed the diagnosis of pancreatic leiomyosarcoma.
The tumor was characterized by its large size and complex internal structure.
